Baptism Registration Form

Parents must be registered, active members of St. Paul parish for at least two (2) months prior to Baptism. Your parish registration number can be obtained from the parish office (386)-252-5422.

GODPARENTS - At least one of the Godparents must be fully initiated (Baptism, First Communion and Confirmed), practicing member of the Catholic Church, married through the Catholic Church and over the age of sixteen (16). Godparents must have a sponser certificate (from his/her parish) and must have attended Baptism Preparation class within the last two (2) years.
